Eligibility: The sweepstakes is open to individuals who are legal residents of the United States and are at least 18 years old at the time of entry. Employees, affiliates, and immediate family members of Parallel Path are not eligible to participate.
Entry Method: To enter the sweepstakes, participants must complete the Health and Wellness Consumer Panel Survey in its entirety. Each participant will receive one entry per survey completed.
Entry Period: The sweepstakes entry period will be clearly specified in the initiating email for each survey. Surveys submitted after the closing of the response window will not be eligible for the sweepstakes drawing. Entry period rules are subject to change.
Random Selection of Winner: The sweepstakes winner will be selected randomly from all eligible entries received during the survey period. The selection will be conducted using a random number/drawing tool to ensure fairness.
One Winner Per Survey: Only one winner will be chosen for each individual survey. The same winner cannot be selected in consecutive surveys. If a previous winner’s name is drawn again in the subsequent survey, a new random selection will be made until an eligible winner is determined.
Winner Notification: The winner will be notified via the email address provided to complete the survey. The winner will have three attempts to sign for and claim their prize. If the winner does not respond within three attempts or is found to be ineligible, another winner will be randomly selected.
Prize: The winner will receive a $125 AMEX gift card (or similar prize). The gift card will be mailed to the winner’s address. Prize will be shipped via USPS Priority Mail.
Gift Card Acceptance: Winner accepts all sweepstakes terms and conditions upon signature of delivered gift card by carrier.
Taxes: All taxes associated with the receipt or use of the prize are the sole responsibility of the winner.
Publicity Release: By accepting the prize, the winner agrees to permit Parallel Path to use their name, likeness, and survey responses for promotional and marketing purposes without further compensation or permission, except where prohibited by law.
General Conditions & Restrictions: Parallel Path reserves the right to disqualify any individual found to be violating sweepstakes rules or tampering with the entry process. The sweepstakes is subject to all applicable laws and regulations and is void where prohibited.

- Survey Participant Location: Survey participants must reside in the United States
Modifications and Termination: Parallel Path reserves the right to modify, suspend, or terminate the sweepstakes for any reason, including but not limited to technical difficulties, fraud, or other issues beyond our control, that may compromise the fairness or integrity of the Sweepstakes.
By participating in the Consumer Insights Collective Survey Sweepstakes, participants agree to be bound by these rules and the decisions of Parallel Path, which are final and binding in all respects.
